ABSTRACT

Objective:To investigate the feasibility and clinical effect of the surgical approach and method of transnasal fenestration under nasal endoscope for the treatment of maxillary odontogenic cyst. Methods:The clinical data of 23 cases with maxillary odontogenic cysts treated by nasal endoscopy through nasal fenestration were retrospectively analyzed. All cases underwent nasal endoscopy and CT examination before the operation. The mucosal membrane of the parietal wall of the cyst was excised through fenestration of the nasal base. The cyst fluid was removed by decompression, and the bony opening of the nasal base was trimmed and enlarged to the edge of the cyst. The intraoperative and postoperative effects were observed. Results:All cases were well exposed under the direct vision of nasal endoscope. The top wall of the cyst was removed to maximize the communication between the cyst cavity and the nasal floor. There were no complications such as nasolacrimal duct injury, turbinate atrophy, necrosis, and facial numbness. All patients were followed up for 6-12 months, and the clinical symptoms gradually disappeared after surgery. The inferior turbinate was in good shape, the cyst cavity was smooth, the cyst wall was determined, and no cyst recurrence was observed. Conclusion:The treatment of odontogenic cyst of maxilla under nasal endoscope through nasal fenestration is convenient. It has less trauma, fewer complications and a satisfactory curative effect, which is worthy of clinical promotion.

ABSTRACT

Objective:To analyze results of microbiological culture of lesion exudates and drug sensitivity test in patients with pemphigus, and to optimize the management protocol of pemphigus.Methods:Inpatients with pemphigus were collected from Department of Dermatology and Venereology, the First Affiliated Hospital of Anhui Medical University from January to December in 2008, and the results of microbiological culture of skin lesion exudates and drug sensitivity test were retrospectively analyzed.Results:A total of 176 patients were collected, and skin lesion exudate culture yielded 25 kinds of bacteria and 11 kinds of fungi in 156 (88.64%) patients. Of the 156 patients, 1 kind of pathogen was yielded in 104 (66.67%), and 2 or more kinds of pathogens were yielded in 52 (33.33%). Gram-positive cocci were detected in 134 (76.14%) cases, including Staphylococcus aureus in 80, Staphylococcus hemolyticus in 21, and Staphylococcus epidermidis in 16; Gram-negative bacilli were detected in 66 (37.50%) cases; fungi were detected in 24 (13.64%) cases. In addition, 89 (93.68%) of 95 relapsed patients showed positive culture results, 67 (82.72%) of 81 patients with newly developed pemphigus had positive culture results, and the positive rate significantly differed between the two groups ( χ2 = 5.22, P <0.05). Drug sensitivity test for Staphylococcus aureus isolates showed that 71 (97.26%) of 73 were sensitive to vancomycin, 69 (97.18%) of 71 to linezolid, and 61 (87.14%) of 70 to rifampin. Conclusion:Cutaneous infection, especially Gram-positive cocci infection, is very common in patients with pemphigus, and Staphylococcus aureus clinical isolates were highly sensitive to vancomycin, linezolid and rifampicin.

ABSTRACT

Objective@#To explore the clinical features, diagnostic methods and therapeutic strategy of silent sinus syndrome (SSS).@*Methods@#A retrospective study was made on eight SSS patients treated during 2013-2016 in Longgang ENT Hospital and Otorhinolaryngology Hospital of the First Affiliated Hospital of SUN Yat-sen University. The following clinical data, including demographic data, symptoms, history of trauma and surgery, signs, imaging examination, endoscopic surgery and postoperative outcomes, were analyzed to summarize the diagnosis and treatment experiences.@*Results@#Eight SSS patients showed the following clinical features: the proportions of both sexes and sinus sides were 4 to 4; seven cases (7/8) were adult, with an average of (48.1±11.8)y; seven cases (7/8) had long history of trauma or surgery, with an average of (17.9±10.5)y; seven cases (7/8) cannot recall the exact course of SSS; six cases (6/8) had no nasal symptoms; eight cases (8/8) had unilateral ocular discomforts; eight cases (8/8) had signs of unilateral enophthalmos (2-5 mm), accompanied with hypoglobus; and by CT and MRI scanning, eight cases (8/8) showed the unilateral maxillary sinus outlet obstruction, sinus full opacification, sinus wall bony rarefaction, sinus wall contraction, sinus volume loss, and the ipsilateral orbital floor bowing descent and orbital volume increase. After treated by endoscopic sinus surgery and followed-up for one year, four cases were cured, and the other four improved; no intra- or post-operative complications occurred; and no individual need a two-stage orbital plastic reconstruction.@*Conclusions@#SSS often develops in the unilateral maxillary sinus of adult patient with long history of trauma or surgery, but the nasal symptoms and signs are silent. Diagnostic for SSS depends on characteristic ocular sign and sinus CT imaging. Endoscopic sinus surgery helps to improve ocular and nasal signs and promote orbital self-reconstruction.

ABSTRACT

Objective To investigate factors impacting on local recurrence in cases of primary retroperitoneal liposarcoma (RPLS) after complete enbloc resection.Methods The clinical data of 47 cases of primary retroperitoneal liposarcoma were retrospectively analyzed.Results 3-year local recurrence rate was 62%.Single and multiple regression analysis showed preoperative needle biopsy,histological subtype,tumor size and capsular invasion were all risk factors related to postoperative local recurrence in primary retroperitoneal liposarcoma undergoing complete resection (P < 0.05).Conclusion Preoperative aspiration biopsy,tumor poor differentiation,large tumor size tumor extracapsular spread all lead to high local recurrence rate after complete resection of primary retroperitoneal liposarcoma.

ABSTRACT

<p><b>OBJECTIVE</b>To explore the possible genetic model of psoriasis vulgaris.</p><p><b>METHODS</b>The complex segregation analysis and heritability calculation were performed with the aid of Penrose method, Falconer regression method and SAGE-REGTL program.</p><p><b>RESULTS</b>It was found that in 1043 patients with psoriasis vulgaris, 305 patients (29.24%) have the family history of psoriasis, and 738 patients have not the family history. A ratio of s/q approached 1/(square root of q) with Penrose method, and the heritability values of psoriasis in the first-degree and second-degree relatives were 67.04%, 46.6% respectively. By complex segregation analysis, Mendelian, non-major-gene model and environment model were rejected for psoriasis.</p><p><b>CONCLUSION</b>The results suggest that psoriasis follows a pattern of polygenetic or multifactorial inheritance rather than single-gene inheritance.</p>

ABSTRACT

Objective To identify a psoriasis susceptibility locus on chromosome4q in a Chinese Han population.Methods The genome search was performed using12microsatellite spanning chromosome4q in64Chinese Han families(372family members)comprising197affected and175unaffected individuals.GENEHUNTER was used for the parametric and non-parametric linkage analyses.Results(1)Non-paramet-ric linkage analyses:single-point analyses revealed that two adjacent loci on chromosome4q D4S413and D4S1597had evidence of linkage,with NPL-scores of2.04and2.23,and P values of0.021and0.014re-spectively.Multi-point analyses showed a peak NPL-score of3.44and the corresponding P value of0.00056at157.9cM where D4S413located.Moreover,NPL score of more than3was found with a range from155.1cM to172.3cM.(2)Parametric linkage analyses revealed a LOD score of3.70,a heterogeneity LOD score of4.35and a high proportion of linked families(?)of85%under the assumption of a dominant model with dis-ease-allele frequency of0.0062and penetrance of10%at D4S1597.Conclusion Chromosome4q contains genes involved in the susceptibility to psoriasis vulgaris in a Chinese Han population.

ABSTRACT

Objective:To investigate the value of spot affinity purified method in the isolation and purification of mono specific basement membrane zone autoantibodies(BMZ Ab).Methods:Utilizing human dermoepidermal antigen transmitted on NC membrane,We purified 3 cases with mono specific BMZ Ab in sera of SABD patients which was oppositive by immunoblotting.Results:The BMZ Ab purified by this method could specifically combine its target antigen.Conclusion:Spot affinity purified method could be widely used in the isolation and purification of autoantibodies produced by mixed micro antigen.
